Client Record Management and Scheduling

Throughout the process, we record your details and needs from the care plan and assessments into our proprietary care software. This enables us to match your loved one with an appropriate caregiver or caregiver team, schedule visits according to your needs and monitor each shift for appropriate coverage.

Person-Centred Assessment and Care Planning

Once all parties agree on an appropriate level of care and to begin home care services, one of our experienced members of staff will visit to deliver the full personalised care plan and assessments and go through in detail with you to ensure that the care plan reflects the care that you want an the outcomes you desire. The care plan will be kept in the home and followed by the caregiver during every visit.

Follow Up, Quality Checks and Reassessments

To ensure that you are receiving the highest quality care, we will call you regularly, during the first and second weeks and every month thereafter to ask if you are satisfied with the caregiver and services. We also provide home visits to spot check our caregivers and improve the services delivered. To continue meeting your goals we will perform reassessments whenever they are required, at least six monthly, to ensure that the care we provide, is the care that you need and we are working towards and succeeding in meeting your goals.

Caregiver Selection

We consider what we have learned about your loved one during our conversations, visit and assessment and then carefully match your loved one with a caregiver or caregiver team that best matches his or her personality and needs. We provide on-going training and education for our caregivers, so they are the most knowledgeable in all areas of home care.

Medication Assistance

All our staff are fully trained in administering medication and can assist you to take your medication from a chemist filled Dossett box. If you have specialised needs you will be allocated a care worker trained in the specialised technique required and your care/support plan will be agreed with others involved in your health care, such as your district nurse who will assess your care worker and sign them as competent.

Companionship

All of our services are flexible and can be tailored to meet your individual needs. Your care support plan will be designed with you to ensure that you reach your desired outcomes. All our care team have enhanced CRB (Criminal records) checks prior to their commencement of work with Radical care and yourselves.
